PCN Mentoring

PCN Mentoring

Short for Primary Care Network, PCN's are the organisational structure of GP Practices. The programme provides tailored one-to-one support for young people (YP) across east London supporting mental health, wellbeing, reduction in negative/violent behaviour, future aspirations and goals, and physical health improvement.

When it is running and where

Barking & Dagenham, Newham, Tower Hamlets, Redbridge, and Havering. Sessions take place at the GP Practices, schools, or The Foundry

Who is it targeting

Young people aged between 11-18 years old. The project can support YP who may be experiencing any of the following: vulnerable/affiliated to violence, knife crime and exploitation, bullying, mental health concerns, disengaging with school, future aspirations and physical health improvement.

Why it will benefit them

Sessions are youth led, designed to be time for them to seek support through non-judgmental spaces and conversations. Young people can spend time with a trusted adult, discussing interests and learning new skills.

How to join

Referrals are made through GP Practice staff (Care Coordinators, Social Prescribers, Nurses, Practice Managers, GP's). We can receive referrals from other community partner (local authority/youth services) and schools. We will then inform the GP Practice.
